Usability Testing

The ideal participant is a qualified Radiation Therapist or Medical Physicist who has used Vision RT products. By taking part and giving your informed consent in the Form, you acknowledge that you represent this population in some capacity.

We (hereafter all references to “We”, “Our” or “us” will mean Vision RT Usability team and all references to “You” or “Your” will mean the participant), hereby mutually agree with you as follows:

1. You are taking part of your own free will and may discontinue the conversation at any time and for any reason. However, your data will still be included up until the point you chose to leave.
2. You will provide us with objective feedback and comments with regard to our products. We own all rights, title and interest in and may use such without reference or payment to You in further product development and testing that We may undertake.
3. You acknowledge that the products that You will be testing, any materials provided to You and the results of the sessions and testing are Our Confidential Information. You agree that You shall take all reasonable measures to protect the secrecy of and avoid disclosure and unauthorized use of Confidential Information. You shall not make any copies of Confidential Information unless We approve same in writing. You will immediately notify Us of any unauthorized use or disclosure of the Confidential Information.
4. The terms of this Agreement and any dispute or claim arising out of or in connection with it or its subject matter shall be governed by and construed in accordance with the law of England and any dispute or claim shall be resolved in the federal or state courts within England, without regard for principles of conflicts of laws.
5. This Agreement shall survive until such time as all Confidential Information disclosed hereunder becomes publicly known and made generally available through no action or inaction of either party to this Agreement.

Privacy notice

All data provided is appreciated and will be kept confidential and in accordance with UK data protection laws.

• Any personal data (information by which you can be identified) you provide to us:
  • Will be kept on file by Vision RT Limited for the lifetime of the medical device the study relates to.
  • Will only be used in association with the study and medical device it relates to. It will not be used or shared for any other purposes.
  • Will not be transferred to any third party without your express written consent.
  • Will only be disclosed to third parties with your express written consent (which you will be able to withdraw) or pursuant to a legal obligation to which Vision RT Limited is subject.
• Any processing of personal data provided by you on this form is necessary for our legitimate interests, namely to study how people interact and use our products/services, and to develop our products/services and grow our business.
• You have the right to lodge a complaint with the UK Information Commissioner’s Office in respect of our use of your personal data.
• You acknowledge that Vision RT Limited is entitled to anonymise the personal data you provide on this form by removing all factors from it by which you may be personally identified, and to aggregate the information you provide on this form with data provided by third parties.
• You acknowledge that to the extent information provided by you in this form does not constitute personal data, or is created by anonymising your personal data or aggregating it with data provided by third parties, Vision RT Limited shall be entitled to use, and to permit third parties to use, such data in such manner as it sees fit.
